Nethralokana Sathkara strives to create a positive change in the field of eye health of our nation that is important to everyone in the country. We are proud to introduce this opportunity for needy immaterial to gender, race, age, ethnic origin, or creed, to request their most wanted personalized spectacle through our Corporate Social Responsibility (CSR) website. Each year, Nethralokana Sathkara will allocate a limited amount of customized spectacles though this portal.

Before an individual is considered for a grant, he/she must fulfill the following criteria:

• The individual should have a recognized visual impairment.
• The individual must be willing to provide documentation to verifying financial status.
• The individual must be located in Sri Lanka
• The individual must be inclusive in policies and practices involving with Nethralokana Sathkara.

If you are interested in submitting a request for a donation, please fill the below application form. You are welcomed to make requests for special eye camps by completing first part of the application, mentioning about your requirement in “Special Notes” column.
